Chapter 2: Assessment

An ongoing process

Building and nurturing trust continues throughout the call. Similarly, assessment is not an endpoint. In the course of a palliative care call, it’s very possible that the agreed upon plan will change based on a rapidly evolving clinical profile. You may find that reassessing and discussing treatment options and goals of care is ongoing because:

  • The patient’s status may be changing quickly.
  • Family members are fearful and tentative.
  • New information may be shared.

A continuing, constructive dialogue with the patient and family will assist you to identify and meet patient and family needs.
